The Rehabilitation Institute of Washington is committed to keeping your private medical information secure. Please see our Notice of Privacy Practices for more information about your rights and our obligations in the use and disclosure of your health information.

To Request a Copy of Your Medical Record:

All requests must be made in writing. Please print and complete our Authorization to Use or Disclose Healthcare Information form. You may return the form by fax or mail.  You may also come to the clinic in person to complete the form.

Although we will strive to complete your request as soon as possible, please allow up to two weeks for processing. All requests will be subject to our current handling and copy charges.  To view our current copying fees, please click here.

For Washington State Workers’ Compensation claims, you may be able to access your medical records online at the Claim & Account Center. Follow the instructions to create an account and find answers to common questions.

For Healthcare Providers:

Please fax your request on clinic or company letterhead to (206) 859-5031.  If the request is urgent, please contact our office directly so that we may promptly assist you.
